Botulinum toxin has been one of the most trusted treatments in aesthetic medicine for decades, helping to soften lines, relax overactive muscles, and create fresher, more rested looking results. But occasionally, patients begin to wonder whether their treatment is "stopping working."

Perhaps the results do not seem as strong as before. Perhaps they wear off more quickly. Or perhaps there seems to be little effect at all. This can lead to understandable concern about something called botulinum toxin resistance.

The good news? True resistance is uncommon — and in many cases, the explanation is far less alarming than it sounds.

What is botulinum toxin resistance?

Botulinum toxin resistance happens when the body becomes less responsive — or, very rarely, unresponsive — to treatment. In simple terms, the toxin no longer works as effectively as expected on the targeted muscles. This may happen after repeated treatments over time, or very rarely, from the very first treatment.

How does botulinum toxin normally work?

Botulinum toxin works by temporarily blocking nerve signals to muscles. Without those signals, the muscles cannot contract as strongly, which reduces movement and softens lines caused by repeated facial expression — such as frown lines, forehead lines, and crow's feet. When resistance occurs, the toxin is either neutralised before it can work, or the muscles simply fail to respond as expected.

What causes resistance?

1. Antibody formation

This is the most well-recognised cause of true resistance. Over time, the body may develop neutralising antibodies that recognise botulinum toxin as a foreign protein and block its effect. This is more likely when treatments are given too frequently, doses are too high, or large cumulative amounts are used over time. Thankfully, this remains rare.

2. Treatments too close together

Repeated treatment without adequate spacing increases the chance of antibody development. This is one reason why practitioners recommend leaving proper intervals between appointments. Botulinum toxin works best with patience — not persistence.

3. Natural variation in muscle response

Sometimes resistance is not true resistance at all. Muscles may become stronger over time, respond differently between cycles, or require adjusted dosing. A stronger muscle does not necessarily mean resistance — it may simply need a different approach.

4. Injection technique or placement

If product placement is not ideal, results may be disappointing even when the toxin itself is working perfectly well. In these cases, the issue is not resistance — it is technique refinement.

5. Product handling issues

Botulinum toxin must be stored correctly, reconstituted properly, and used within appropriate time frames. Improper preparation can affect potency.

Signs that may suggest resistance

Possible signs include little to no visible improvement after treatment, effects wearing off much faster than usual, and areas repeatedly failing to respond despite correct dosing. However, these symptoms alone do not automatically mean resistance. They simply mean further assessment is needed.

What should happen if resistance is suspected?

A good practitioner will investigate carefully rather than jump to conclusions. This usually involves reviewing treatment history (dosing patterns, frequency, previous response trends), reassessing muscle activity, and checking technique.

Can switching brands help?

Sometimes, yes. Different botulinum toxin brands may work differently in resistant patients, including Xeomin, Dysport, and Azzalure. Xeomin is often considered useful because it contains no accessory complexing proteins, which may reduce antibody stimulation.

Alternatives to botulinum toxin

Even if resistance develops, there are still excellent alternatives available.

Can resistance be prevented?

While not always preventable, the risk can be minimised. More frequent injections are not better — and can sometimes create the very problem patients are trying to avoid.

Botulinum toxin resistance is real — but rare. Most cases of reduced effectiveness are caused by far more manageable factors such as muscle strength, dose adjustments, timing, or technique. If treatment seems less effective than expected, the most important step is not to panic — it is simply to have an honest discussion with your practitioner.
